KOS-BAR – Kompleksowa Opieka Specjalistyczna Bariatryczna (Comprehensive Specialist Care in Bariatrics), Poland
KOS-BAR is delivered through a centralised, multidisciplinary care model within a single facility that ensures effective coordination at every stage of treatment, including monitoring of treatment outcomes and the entire therapeutic process. The aim and objective of comprehensive specialist care for patients with morbid obesity is to improve the effectiveness and quality of patient treatment. The pilot programme is aimed at patients aged 18 and over diagnosed with obesity caused by excessive energy intake, who have a BMI of at least 40 kg/m² or 35–40 kg/m² and have comorbidities, and for whom surgical treatment may offer potential benefits.
The pilot programme comprises four modules Diagnosis and pre-operative management, Surgical treatment, Medical rehabilitation (pre- and post-operative), Specialist bariatric care (monitoring).

CureApp, Japan
CureApp is developing „Digital Therapeutics (DTx)” in order to treat conditions that have not been effectively treated by conventional therapies. Different from the conventional treatment methods with drugs and devices, DTx apps prescribed by healthcare providers focus on behavior modification. We strongly believe that therapeutics using mobile applications will soon become the standard method of treatment.
Recently, ICT-based (Information Communication Technology) initiatives are gaining popularity in the occupational health field. However, not all initiatives provide valuable evidence-based support. CureApp developed the „ascure mobile health program” for private corporations with a mission to „deliver effective health solutions for disease prevention based on high-level evidence.” For the program, we fully applied our knowledge and experience in developing therapeutic applications.

HealthyW8 – Healthy Lifestyle Recommender System (HLRS), Luxembourg
HealthyW8 is a Luxembourg-coordinated digital obesity-prevention initiative built around a Healthy Lifestyle Recommender System (HLRS). The project website states that it develops a digital-based recommender system tailored to individual needs and preferences. It combines eHealth interventions, gamification, mood and psychological aspects, and support from the HLRS and an emotions-aware Human Digital Twin (HDT). The scientific pilot protocol further describes the intervention as including a mobile app for meal planning, gamified physical activity support, and real-time monitoring through a wearable smartwatch. The HDT is intended to forecast behaviour and support simulation and prediction for individual decision-making.

The Turkish Personal Health Record system e-Nabiz allows users to access 30 different services for treatment, prevention, health promotion, and health-related areas. By 2023, 4.5 billion transactions had been performed, with 220 million physician queries. The system is adopted by 82% of the Turkish population. e-Nabiz displays BMI directly on the personal health record homepage, provides information on cities with highest obesity rates, and lists obesity physicians, enabling patients to proactively seek specialist care.

Better4UAll operates through an integrated digital health platform that brings together mobile technologies, data analytics, and behavioural science to support obesity prevention. The system collects user data either actively, through self-reported inputs, or passively, via mobile applications, wearable devices, and online tools, capturing information on physical activity, dietary habits, and broader lifestyle behaviours.
These data are then aggregated and processed within a central cloud-based infrastructure, where advanced analytics and AI algorithms are applied to identify behavioural patterns, risk factors, and user-specific needs. Based on these insights, the platform delivers personalised recommendations and behavioural nudges, aiming to encourage healthier choices and sustained lifestyle changes.
At the same time, continuous monitoring enables real-time feedback to users, enhancing engagement and self-management, while also generating aggregated data that can inform public health authorities and practitioners. In this way, Better4UAll moves beyond generic interventions by providing a dynamic, data-driven approach to obesity prevention that is tailored to individual and population-level needs.

Omada Health is a digital health platform that delivers lifestyle intervention programs through a mobile app and web interface. Users enroll online and receive connected devices (e.g. wireless scale) that automatically sync health data such as weight and activity. The platform analyzes these data to provide personalized feedback, goal setting, and progress tracking. Participants engage in structured educational modules and communicate with health coaches via messaging and telehealth tools. Data analytics and behavioral science are used to support continuous monitoring and tailored interventions, while AI is applied in a supportive role to enhance personalization and user engagement.

Beyondbmi is a digitally delivered Irish medical weight-management programme. Users begin with an online eligibility survey, then complete onboarding through a secure member account, download the Beyondbmi app, and book video consultations with doctors trained in obesity management. The service provides ongoing multidisciplinary support and allows members to track changes in sleep, mood, weight and more through the online platform. The programme is designed for long-term support rather than one-off prescribing and includes continued access to doctors, dietitians, and health coaches through a 12-month digital pathway.

Terveyskyla’s services were originally developed as a collaboration between all university hospitals in Finland. Today, the services are provided by several university hospitals and wellbeing services counties. HUS acts as a producer of Health Village’s ICT services. Terveyskyla is a public online service. Its contents are produced by social welfare and health care professionals in cooperation with service users. The Health Village consists of three services: Health Village website open to all, i.e. the Health Village hubs, Digital service channel My Path, and Professionals’ service portal HealthVillagePRO. Digital therapeutic intervention with digital program Vitadio, Czech Republic
Typically program lasts 6 months, divided into an intensive 3-month phase followed by a sustaining phase. Users receive daily tasks and educational content that evolve based on their individual progress and choices. SMART (Specific, Measurable, Achievable, Relevant and Time-Bound) goals are created to encourage healthy habits in nutrition, physical activity, sleep and mental wellbeing. The app features an AI tool (ALFRED) for taking a photo of their meal to identify ingredients and provide real-time nutritional feedback. Patients use the app to track their daily routine: photo-based food diary, pedometer data, blood glucose levels and weight. The program also provides access to a personal dietitian via chat for technical support and guidance. Additionally has a peer support community to increase motivation.

PhaseV – obesity proof-of-concept study, Denmark
PhaseV is developing a digital model for decentralized clinical studies that collects patient-generated real-world data through questionnaires and connected devices, presenting insights to participants in an app and to clinicians in a web portal, both developed by Monsenso. In the obesity proof-of-concept study, Steno Diabetes Center Aarhus is running a 24-week, fully online study in which participants receive support from clinical dietitians while data are collected at home using a smartwatch, connected scale, and study app. The broader PhaseV project is designed to support long-term monitoring and real-world evidence generation in obesity and other conditions.

Estonia’s EHR is a nationwide system integrating data from all healthcare providers into a common record accessible online. The system’ technology guarantees data integrity and prevents tampering. OBCT delivers: (1) a holistic obesity risk screener for the public and health professionals; (2) country-specific trend estimates; (3) a digital interactive atlas mapping the 'obesogenicity’ of local environments across the EU; (4) decision support dashboards for policymakers; and (5) co-developed toolboxes for implementing policy in low-socioeconomic communities. The Amsterdam UMC team compiled an obesity risk map based on environmental characteristics of each neighbourhood, serving as the template for the EU-wide digital atlas. AI and data analytics underpin the risk screener and environmental scoring models.

STop Obesity Platform (STOP) project aims to build an innovative platform to support persons with obesity with better nutrition under the supervision of healthcare professionals. STOP Fitness Manager supports users in the task of creating and managing their fitness data. They consist of several modules that help to manage users’ activities, exercises, food and water consumption, and weights. After completed registration and login using username and password, users are required to create a fitness profile. This step is needed for the system to calculate a personalized fitness plan for each use. Additionally it enables access to information from a broad range of relevant knowledge bases to support decision making of Healthcare Professionals. An innovative platform that combines various data from people with obesity based on sensor streams, chatbots with existing knowledge databases, merging them by using machine-learned data approaches for sophisticated AI data analysis.

The National Childhood Obesity Prevention Program in Greece uses a centralized digital platform to collect and manage data from schools and primary healthcare services. Anthropometric measurements (e.g., BMI), dietary habits, and physical activity levels are recorded by trained professionals and entered into the system.
The service operates as a digitally supported, integrated public health programme combining online access, clinical pathways, and community-based interventions. Through the official platform, users (parents and children) access information, register, and are guided to appropriate services. Children undergo clinical assessment by healthcare professionals (e.g. paediatricians), who evaluate growth, nutritional status, and risk factors, and provide referrals for further support.
The system enables coordinated care by linking healthcare services with schools and community programmes, while also offering remote support such as teleconsultations with dietitians and multidisciplinary teams. Personalised guidance (nutrition, physical activity, lifestyle) is delivered through structured sessions and educational content.
At a system level, data collection and monitoring support evidence-based decision-making and national tracking of childhood obesity trends, enabling continuous evaluation and improvement of interventions.

Kraftmiklir krakkar (“Strong Kids”), Iceland
Kraftmiklir krakkar is an Icelandic lifestyle clinic for children with obesity and their parents, developed within HSU primary care. Children in selected school grades were screened through school-health procedures, and those identified with obesity were invited with their parents into a structured six-month intervention. According to HSU, the model included six consultations over six months, mostly with nurses and one to two with a physician. Public reporting also states that the project examined the feasibility of in-person versus remote consultations, and that remote consultations worked well.

SLIMMER Online is a digitally enabled Dutch lifestyle intervention for people with overweight or obesity. It combines 5 hours of personal coaching with 26 weekly online group sessions that participants follow from home using the Ancora app. Participants are referred through their GP or practice nurse, who provide recent weight and waist measurements. The wider Ancora platform supports total health tracking, personal trends and insights, and data-driven care delivery. A Dutch pilot study of Ancora’s digitally enabled lifestyle intervention reported that participants could use the app to register and track health behaviors, weight, and anthropometric data over time. The initiative therefore combines app-based self-monitoring, remote coaching, and structured online support for weight management.

It is accessed via smartphone, tablet, or computer, and offers digital weight-management support through approved providers. NHS England states that participants are referred by GPs or community pharmacies, and that the service includes different levels of support, from self-guided digital content and group support to one-to-one coaching. GP referrals are made through EMIS, SystmOne, or Medicus e-referral systems, making the initiative a practical example of digitally enabled obesity care linked to routine primary-care workflows.

The campaign was created in support of the National Program for the Prevention of Obesity in Children and Adults, with the aim of promoting positive lifestyle changes and highlighting the importance of balancing all aspects of life. HealthyW8 – Personalized Obesity Prevention Platform is a dynamic online space that serves for a wide range of information and tools related to obesity prevention and treatment.

HealthyW8 incorporates different technologies, including mobile components (like smartphone apps and wearable device integration). For example, “HealthyW8 Watch” apps can collect fitness data (steps, calories, stress) from connected devices via Bluetooth and feed them into a user profile to inform recommendations.
The project integrates biological data (e.g., non‑invasive bio‑marker measurements such as saliva samples) to link physiological aspects with lifestyle and health outcomes.
A central piece of the project is a digital system that generates tailored lifestyle advice based on each user’s context, preferences and physiological data. This system is designed to provide recommendations for nutrition, physical activity and other health‑related aspects rather than generic advice.
The project develops Human Digital Twin technology — a dynamic computer model representing individual characteristics.It is used to simulate and predict how different interventions might affect a person’s health outcomes.
